This video presents a clinical case of a heavy smoker patient with Rutherford scale III symptoms on the left side, who has already been treated on the right limb. The case is characterized by a complete occlusion of the distal portion of the superficial femoral artery. The patient was evaluated using IVUS, followed by atherectomy, drug-coated balloon angioplasty, and finally, stent placement.

Educational Objectives

  • Understand the indications and principles behind angioplasty, including when to use IVUS, atherectomy, drug-coated balloons (DCBs), and stenting.
  • Learn to use IVUS for real-time imaging to assess lesion characteristics and complications to optimize angioplasty outcomes.
  • Identify appropriate use of atherectomy for calcified lesions and DCBs for reducing restenosis, particularly in high-risk patients.
  • Understand the use of stenting in angioplasty related complications.

Step-by-step procedure:

After angiographic evaluation of the lesions, a 0.014” guidewire was used to navigate through the occlusion with IVUS guidance.

Once the occlusion was successfully crossed, atherectomy was performed.

Treatment with a drug-coated balloon (DCB) was then performed.

Post-treatment angiography revealed a severe dissection in the treated area, which was confirmed by IVUS.

A self-expanding stent was selected and placed to treat the dissection, resulting in good angiographic outcomes.
